Felicity Holistic Care partners with their clients, ensuring they can choose how they are supported according to their unique needs. At Felicity Holistic Care, “Happiness” is at the very heart of what we do and what we want all of their clients to experience in their lives. They also work with the Department of Families, Fairness and Housing (DFFH) to provide care and services for children and young people at risk. 

About the Opportunity

We are seeking an experienced Support Coordinator who is enthusiastic, passionate, and caring to join our team supporting people living with disabilities. Using your exceptional research and analytical skills, you will work with the clients to implement the supports to build capacity, manage the impact of their disability and improve their health, well-being and participation within the community.

In this role your responsibilities will include:

  • Working with participants to achieve their goals in line with their NDIS plans, including encouraging and facilitating client inclusion in community activities
  • Establishing strong relationships with clients, internal and external stakeholders
  • Managing and maintaining client plans and activities
  • Monitor changes within the NDIS and Child Protection, record and analyse data relevant to service delivery
  • Monitor and provide quality of service delivery, while ensuring compliance
  • Use a coaching approach to assist participants in capacity building
  • Working in a respectful way with people with varying support needs, their families, support networks and community
  • Supporting evidence-based practice through your high level of administrative skills, documenting outcomes for individual NDIS participants, evaluating service delivery and identifying challenges and opportunities in the new environment
  • Creating documents and complex reports, constructing proposals, and funding submissions

To thrive in this role, you will bring the following: 

  • Relevant qualification and/or extensive experience in allied health, disability, or community services
  • Experience as a NDIS LAC/Planner or Support Coordinator highly regarded
  • A strong understanding of the disability sector 
  • Strong commitment to principles of enhancing independence and client empowerment
  • Experience coordinating specialist services for those with complex needs working with cross-functional teams
  • A positive attitude with a flexible and adaptive nature to respond to the changing needs of our clients
  • Excellent project management, communication, and interpersonal skills. 
  • Happy to travel up to an hour to see clients
  • High attention to detail to produce a high-quality standard of work, including report writing skills
  • Demonstrated ability in maintaining organisational compliance.
